New York Mayor Bill de Blasio on Tuesday issued a bold guarantee of affordable health care for every resident, thrusting the nation's largest city to the forefront of debates over universal health coverage and immigrant rights.

The new NYC Care plan, which de Blasio said will be funded without tax increases, is an expansion of the city's existing MetroPlus plan covering hospital bills for low-income residents.

NYC Care, de Blasio's new program, will provide coverage to the New Yorkers who are now using the emergency room as their sole health care provider.

During a news conference at a Bronx hospital that is part of the city's network of public hospitals and clinics, de Blasio said repeatedly that he would prefer a federal or state single-payer health care system.

De Blasio explained he wants the plan to stop uninsured New Yorkers from using the ER as their primary care provider, and believes NYC Care will actually save the city money. The city said last week that an ambitious goal of providing MetroCards to about 800,000 residents whose incomes are below the federal poverty line would start this month with cards for only about 30,000 people, or about four percent of the total. "It's being treated in a language you can understand by people who actually care about your health and well-being".

Residents will be able to access the program via the city's website or calling the city of NY at 311.

De Blasio seems unfazed by the idea that this might be a very expensive boondoggle, of course.

"Health care is a right, not a privilege reserved for those who can afford it", de Blasio said in a statement. No one should go without the health care they need.

"New Yorkers need a break!" he declared, sarcastically dismissing concerns that the policy might have negative ramifications.

The city's plan is similar to programs that have been established elsewhere like the Healthy San Francisco initiative but is broader, said Katz, who has previously led health agencies in San Francisco and Los Angeles.
